Each row is checked once an hour, with one authenticated call to every provider that serves it — image generation, for example, is checked against both hosts that front the same model. The percentage is the share of those hourly checks that passed, out of however many were collected, so it is a reachability record rather than a count of successful renders. A row we cannot check reports “not checked” instead of a green default.
